The Adolescent and Young Adult Nurse Care Coordinator is an experienced Oncology/Haematology Registered Nurse who specialises in the coordination of care for people aged 15-24 years with a cancer diagnosis. The AYA NCC can assist by; being responsive and supportive of the patients individual needs, providing assistance with organising appointments for treatment,
assisting the patient's family to understand the cancer diagnosis and treatment plan, and promoting communication between all members of the health care team. The AYA NCC can be contacted at any point through the cancer journey. Patients can self refer or be referred by a health care professional to the AYA NCC.
